TRIAGE INFORMATION – PLEASE READ CAREFULLY

• The GP can only assess your request based on the information you provide, so please be clear and include as much detail as possible.

• Although this is not an emergency service, the GP may request the same day appointment.

• If you are unable to attend a same day appointment, should the doctor request this, please clearly indicate this in your triage form.

• Please keep your phone nearby in case we need to contact you by call or text. 

• If you would prefer to see a specific clinician, please state this in your triage form and we will try and accommodate your request where clinically appropriate.

• For a nurse appointments, please contact reception.

• For test results, you can either check your online account or call reception.

⚠️ If you feel your health concern requires a same-day appointment, please submit your triage form, where possible, before 12 noon and state this clearly on the form. Requests submitted after this time may not be reviewed until the next working day due to exceptionally high demand.
