Important information about triage
Although this is not an emergency service, the GP may request that you attend an appointment on the same day.
If you are unable to attend should the doctor request this, please clearly state this in your triage form.
The GP can only base their assessment on the information provided in your form. It is therefore essential that you are specific and clear about what you are requesting or needing. Please provide as much detail as possible when answering each question to help the GP triage your request effectively.
Please keep your phone with you in case the GP or the surgery needs to contact you by telephone or text message, or if further information is required.
Submitting your triage request
The preferred method of submitting a triage request is via the NHS App (link below).
⚠️ If you feel your health concern is something that requires a same‑day appointment, please ensure you submit a triage request before 12 noon. Requests submitted after this time may not be reviewed until the next working day due to exceptionally high demand.
